Our "Book an Appointment" page directs you to a scheduling tool provided by our third-party electronic medical record (EMR) provider. When you click to schedule an appointment, you leave our website and enter a system operated by the EMR Vendor.

Any information you provide through that scheduling tool (such as your name, date of birth, contact details, or reason for visit) is collected and processed by the EMR Vendor on our behalf, under a Business Associate Agreement and in accordance with their privacy practices and applicable law, including HIPAA. Information you provide for the purpose of scheduling or receiving care becomes part of your medical record and is protected as described in our Notice of Privacy Practices (see "HIPAA" below).
